Combine sugar, cocoa, cornstarch, and salt in a saucepan using a silicone spatula or wooden spoon.
Slowly add the milk and stir continuously until the mixture is smooth. Constantly stir over medium-low heat, making sure to scrape the bottom, sides, and corners of the pan, until the pudding thickens, and coats the back of the spoon or spatula, approximately 5 minutes.
Add the chocolate and stir until the pudding achieves a very thick and smooth consistency. Remove from heat and transfer the pudding into a serving bowl.
Serve warm, at room temperature, or chilled; it will thicken further as it cools.
